Cancellation Policy

In signing this document, you agree to the following terms and conditions. I understand that training, practicing, or participating in this program could be dangerous for my child if instructions are not followed. I understand that my child must always abide by the rules of Ethos Jiu Jitsu and any instruction provided to my child by Ethos Jiu Jitsu and those instructing my child. It is my duty as a parent/guardian to instruct my child prior to class to seek clarification before proceeding if those rules or that instruction are not understood. I acknowledge that during all such times whilst my child is on the premises in which Ethos Fitness & Lifestyles operates, both their property and their person shall be at my own risk and I will not hold Ethos Fitness & Lifestyles or its instructors and staff liable for any personal injury or loss of property, whether caused by Ethos Fitness & Lifestyles, its staff or agents or otherwise. I understand that if my child fails to comply with instructions provided to them, or chooses not to participate in the class, the child will be excluded from participation and no refund will be given. I warrant that my child is physically and medically sound to proceed with a normal course of exercise I give Ethos Fitness & Lifestyle the rights to publish photos and/or videos of my child. In the event of an emergency Ethos Fitness & Lifestyle reserves the right to call an ambulance and cannot be held accountable for costs involved nor for any other ramifications of taking such action.
